There are different levels of care available for Hospice of Texas patients. At Hospice of Texas we will meet the needs of our patients and families at the location that is most appropriate and comfortable. As our patients’ needs might change, so might the level of care. We will work together with our patients, families, and patients’ personal physicians to determine and smoothly transition any change level of care. The available levels of Hospice care are:
- Routine Home Care: Most of our patients choose to remain in their own home where they are with family and friends who love them, and in the most familiar and comfortable place they know. Our team will meet the needs of the patient and their family in this familiar setting which will include scheduling regular visits by team members to ensure that all physical, emotional, and spiritual needs are met.
- General Inpatient Care: If symptom management cannot be controlled at home. The patient will be transported to a hospital or other inpatient facility. Care will include daily visits by a Hospice of Texas team member to ensure continuity of care, and coordinate care with the facility and Hospice of Texas staff. There will be no additional expenses to our patient or his/her family.
- Respite Care:. If the caring of a loved one with a terminal illness becomes too stressful, or if the caregivers needs a rest, Hospice of Texas will arrange professional caregivers to care for the patient for up to 5 days in a facility contracted for that purpose. This level of care is directed to family members.
- Continuous Care: This care may be necessary if a patient has a medical crisis that requires close medical attention. If this should occur Hospice of Texas staff can provide around-the-clock care in the home. When the crisis is over the patient returns to routine hospice care.
- Professional Care: Patients can be cared for in long term facilities, nursing homes, assisted living centers, and private care homes. Hospice of Texas staff will provide the same quality of professional care wherever our patients call home.
